Information needed when requesting a Certificate of Insurance

For proof of Property and Liability Insurance contact Gabby Reissland at reissland.1@osu.edu with the following information.

  1. Certificate Holder Information*
        i. Name (of whom is requesting the Certificate)
        ii. Company (if applies)
        iii. Address
  2. *This information will appear on the Certificate.
  3. Description of the Event/Reason for Certificate, including your department name and contact information.
  4. Dates
  5. Location
  6. If any additional insureds need to be listed on the Certificate. This would be required only if the other party requires of us in the contract
  7. Name(s) and email address(es) of those requiring the certificate, including certificate holders and department personnel.

For proof of Automobile Insurance contact Bonnie Long at 614-292-6122 in the Transportation Department.
